Reimbursement Guidelines & Post-Activity Protocols
Please read the following instructions carefully. Adherence to these guidelines is mandatory for reimbursement and follow-up procedures.
Proposal Form
- Mandatory for Reimbursement
- Must be submitted 5–7 days before the activity.
- Late submissions will not be eligible for reimbursement.
Summary Form
- Must be submitted within 4–5 days after the activity.
- Required for: Reimbursement processing, Webpage updates, Marketing visibility.
Certificate Request Form
- To be submitted only after the activity has been conducted.
- Early or premature submissions will not be processed.

Every positively diagnosed patient during a camp must be referred for follow-up treatment and properly counseled at the site of camp:

Refer patients to Government Sector Hospitals or DHQ Hospitals
offering free hepatitis treatment and care.
