Reimbursement Guidelines & Post-Activity Protocols
Please read the following instructions carefully. Adherence to these guidelines is mandatory for reimbursement and follow-up procedures.
Proposal Form
- Mandatory for Reimbursement
- Must be submitted 5–7 days before the activity.
- Late submissions will not be eligible for reimbursement.
Summary Form
- Must be submitted within 4–5 days after the activity.
- Required for: Reimbursement processing, Webpage updates, Marketing visibility.
Certificate Request Form
- To be submitted only after the activity has been conducted.
- Early or premature submissions will not be processed.
VFAHT Camp Guidelines: Resources, Registration & Return Protocols
To support your Hepatitis Screening, Safe Blood Donation, Medical, Dental Camps, IPAC FOUNFATION HEAD OFFICE provides the following resources based on outgoing camp volume and inventory availability:
- Hepatitis B RTD Kits
- Hepatitis C RTD Kits
- HIV RTD Kits (only if patient is Hep C positive)
Note: These are Ready-to-Use (RTD) rapid testing kits for efficient and accurate on-site screening. Hep C boxes contain buffers, and these buffers are utilized for both Hep B and Hep C test kits.
IPAC FOUNFATION HEAD OFFICE provide a complete package of essential supplies, including:
- Gloves, Sanitizers, Lancets, Alcohol Swabs
- Screening Cards (for positive patients only)
- Yellow Waste Bags
- Buffers (for kit processing)
- Tongue Depressors (for dental camps)
- Certificates (for volunteers/participants, post-camp)
Note: Any camp supply not available from our end may be purchased by the council and will be reimbursed.
- A template registration form is provided below or a Google link will be shared to you before camp Post-Camp
- Please TCS all unused supplies and leftover kits back to the VFAHT Head Office once the camp is concluded.

- To discuss your camp opportunity, please ensure the camp proposal form is submitted at least one week in advance so that your supplies can be dispatched on time.
Note: Delays in dispatch or delivery may occur during the monsoon season or due to other unforeseen circumstances. We appreciate your understanding and cooperation.
